2016-2017 SCHOOL HEALTH REQUIREMENTS FOR ATTENDING SCHOOL

For Preschool and Kindergarten:

A.      Updated Kentucky Immunization Certificate

B.       Medical Examination (one year prior)

C.       Eye Exam by optometrist or ophthalmologist

D.      Dental Screening/Exam

E.       A Legal Birth Certificate

F.       Social Security Number/Card

 

For all 6th Graders:

A.     Physical Exam-within one (1) year prior to entering 6th grade

B    Updated Kentucky Immunization Certificate

 

For all students participating in sports (including cheerleading and dance teams)

A.      Physical Exam-within one (1) year prior to playing a team sport sponsored by the school. Please check with the Coach for appropriate form.

B.     Updated Kentucky Immunization Certificate

 
Just a reminder:
·       If your child has a condition that requires medication to be given at school, notify the school secretary for the proper forms that are needed so that your child can receive the medicine at school.
 
·       If your child has a specific medical condition that the school needs to be aware of such as diabetes, asthma, seizures, allergies to bee stings requiring a special shot to be given please notify the school secretary.
 
·       All immunizations must be listed on certificate and must have an expiration date before certificate is valid, we cannot accept certificates with one or two shots listed. 
 
·       Physicals must be on the proper form before they will be accepted.
